The Community Flex plans are available through American Community Mutual Insurance Company www.american-community.com American Community Mutual provides high and low deductible health plans with unlimited $30 outpatient doctor office visit co pays. Outpatient doctor office visits are covered on a first dollar basis with no deductible to be paid first. Unlike any other individual health insurance product on the market today, the Community Flex plans include a $1,000 annual Preventative Care benefit per insured. This Preventative Care benefit covers Immunizations, Routine Physical Exams, PSA testing, Routine Mammograms and Routine Pap Smears. These charges would also be covered on a first dollar basis with no deductible to be paid first. HPV Immunizations, Bone Density Tests, Colorectal Cancer Screening & Lab work sent to an independent lab are also fully covered expenses, but not on a "first dollar" basis. The "Complete" comprehensive PPO Health Insurance plan is underwritten by Guarantee Trust Life  Insurance Company, Glenview, Illinois licensed in 49  states and founded in 1936. The Complete plan provides $1,500 to $7,500 calendar year deductibles to choose from. The plan also offers unlimited "first dollar" outpatient doctor office sick visit co pays. With this option selected, each office visit will cost only the $40 co pay with no deductible required. 80/20, and 50/50 co-insurance plans are available. An outpatient co pay prescription drug card is also available which includes $15 co pays for outpatient Generic medications (no deductible required) and $30 co pays for formulary Brand name medications after a $500 calendar year deductible is satisfied. Routine Exams (including coverage for Lab tests - blood/urine) associated with the exam are also covered after 365 days of policy ownership on a "first dollar" basis up to $100 per examination. In addition, Mammograms, PAP tests and Colorectal cancer screenings are also covered from day one with no deductible required if the "Enhanced Routine Preventative Care" benefit is selected. 4th Quarter Deductible Carry over is also included with the plan as well as an Accident Medical Expense (A.M.E.) plan that will cover the first $1,000 of each accidental injury on a "first dollar" basis. Or you can upgrade the A.M.E. benefit to $5,000 of "first dollar" coverage per accident.

Assurant Health Plans www.assuranthealth.com (formerly Fortis Health) www.fortis.com provide high and low deductible plan with $35 unlimited & limited outpatient doctor office visit co pays without first paying the calendar year deductible. 100%, 80/20 and 50/50 co-insurance plans are available. R/X card are optional with a $15 co pay for generic medications with no yearly deductible and $25 co pay for brand name medications after a once a year $500 deductible has been satisfied. Well care is included immediately with a $750 per person allowance yearly. $500 well care benefit on the Core Med plan after 12 months of continuous coverage. Complete Care & Personal Health plans are administered by I.A.C. Insurers Administrative Corporation, Inc. Complete Care Personal Health Plans provide high and low deductible plans with $35 unlimited doctor co pays without first paying the hospital deductible. With 80/20 and 70/30 co-insurance plans available, discount drug cards are included at no additional cost. Co pay R/X cards are optional with a $15 co pay for generic prescriptions drugs with no yearly deductible first, and $30 co pays for brand name, with a separate $250 yearly deductible paid first. A $250 well care benefit per insured is included immediately including yearly mammograms;  you pay only your co pay. Standard Security Life also offers the "High Deductible Health Plan". This plan is one of the most competitively priced H.S.A. qualified H.D.H.P.'s on the market today which includes a "first dollar" mammograms & $250 of "first dollar" Preventative Care benefits.
