The most popular Small Group plans are the Humana PPO 08 Traditional plans. These plans vary by State and may include more or less coverage depending on your specific State health insurance mandates. The Autograph Individual Health plan series includes two different HSA qualified HDHPs. One that includes coverage for outpatient Prescription drugs and one that does not. The Small Group Health Insurance plans also offer Vision, Dental & Life Insurance.
